Preparing for Your Appointment

To ensure your first appointment goes smoothly and efficiently, it’s important to come prepared. Please review the below four items, we will need these for your appointment to better understand your medical history, verify your information, and provide the best care possible. Here are four essential steps:

1. Please bring along a valid ID and insurance card.

2. Our staff will email you your new patient forms.

3. After completing the forms you can email them back or bring a printed copy on the day of your appointment. If you do not get a chance to do so, we ask that you arrive 15 minutes before your scheduled time to fill out in office.

4. Please bring any previous imaging. Your images can be obtained from you’re your previous physician or the imaging center. We accept CDs of the MRI or XRAYs. If possible, bring the image reports as well.

In order for SmartLife to send your medical records to another facility, we must have a signed medical record release from the patient or guardian. Once on file, we are happy to fax records as requested. The medical record forms are available at our office upon request.

Deciding if a SmartLife A2M/PRP procedure is right for you be gins with learning if you are a candidate for treatment. Your initial consultation will include evaluation, diagnostic ultrasound, and a discussion of your candidacy for treatment.

Your physician will make recommendations based on your individual case.

Regenerative medicine uses a patient’s own cells and delivers it to an area of injury.

At SmartLife Medicine we use a high dose, high concentration platelet rich plasma system. We call it A2M/PRP. This concentrated use of platelets, growth factors, and proteins are injected directly into the site of injury by using ultrasound guidance.

During your initial visit, your physician will evaluate your injury, perform a diagnostic ultrasound, review imaging (MRI or XRay) and develop a plan of care.

At SmartLife Medicine, we pride ourselves in taking the necessary time to get to know the patient and learn about their condition. Initial consultations are scheduled for a full hour to allow time for thorough examination.

Your medical insurance plan may cover evaluation and office visits, imaging, physical therapy, and bracing. We recommend speaking with your insurance carrier to verify any benefits and coverage you are eligible for.

Yes, we accept Medicare. While not every service we provide is covered, Medicare does cover a significant portion of our treatments.
